New Patients


Step One

You will need to bring with you, 1 document as proof of address. We accept the following documents as proof of Address:
  • Utility bills 
  • Bank Statements
  • Home office letters
  • Council Letters
AND

1 Document as proof of ID, we accept the following documents ad proof of ID:
  • Passport
  • UK Driving Licence
  • Home Office Card with Photo
  • Birth Certificate for children only. For children under 6, please bring red book or Immunisation record
You do not need to book an appointment to produce your documents. You can present your documents at the reception desk at the following times;
Monday 8.30am - 8.00pm
Tuesday 8.30am - 6.20pm
Wednesday 8.30am - 6.20pm
Thursday 8.30am - 8.00pm
Friday 8.30am - 6.20pm

The reception time is very busy in the mornings therefore we advise all new patients to come to the practice from 10.00am in order to avoid waiting around too long. For our working patients we are now open later on Mondays and Thursdays.

All proof of Address’ must be no older than 3 months from then the date that you come into register.

If you do not have a proof of Address or ID, we will not be able to register you, so please make sure that you bring in your documents to avoid disappointment.

You can also call us on 020 8553 7440 for any query regarding you documents of proof prior to coming into the practice.

Once you have produced your documents, we will print a copy of these and they will be attached to your Registration Form, which will be given to you once the documents have been checked.

We do not keep any of your personal details on our system until you are fully registered and have had a Health Check. You will also be given a date to come back and have a health check before you leave.

Step Two

When you arrive for your Health Check appointment, you will need to make sure that your registration form is fully completed. All sections of the Registration form must be complete and if you have been registered with a GP before in the UK, you will need to provide us with your NHS Number.

This is to ensure that you are seen on time for your Health Check, as an incomplete form may result in a delay for your Health Check and in extreme circumstances, you may not be seen and will have to re-book for another Health Check.

Your Health Check will be done by a Health Care Assistant who is fully trained to measure, height, weight and your blood pressure. This appointment will take no more than 10 minutes. We advise patients to arrive at least 10 minutes before their Health Check time so that any amendments that need to be made can be done so before the appointment time.

If you are on any medication at the time of your registration, please bring along your medication so that the Health Care Assistant can log your medication on the system.

Unfortunately, we do have some extenuating conditions which prevent us from registering patients. We apologise for not being able to register you for the following reasons:
Without proof of Address and ID
If you are a visitor in the United Kingdom and will be staying for 6 months or less.
If you live outside the E12 area.
Any children under the age of 16yrs who does not have a parent/guardian registered at the practice already.
Any children under the age of 6 who’s parents/guardians cannot provide an immunisation record or Red Book issued at birth.

Visitors

We will not be registering overseas visiting patients as they are not entitled to free NHS care. If you would like to be seen by the GP you will need to attend the Walk-in centre at Newham University Hospital or enquire downstairs (ground floor) with the Transitional Team.
